Termination Agreement for Frank Cady

The substantive TERMINATION terms of the agreement, under Section 11,  between Frank Cady and Lassen Municipal Utility District are as follows:

1. Frank Cady agrees to resign as of  ___________________, 2007

2. The District shall not pay Frank Cady the amount remaining on his contract..

3 Frank Cady shall not be entitled to remain on the district's health insurance plan ..

THIS AGREEMENT IS ENTERED INTO KNOWINGLY, FREELY, WITHOUT ANY UNDUE DURESS OR COERCION
